We provide at home nursing services customized to your personal medical needs. All care is provided by an experienced Registered Nurse and coordinated with your healthcare provider such as your Doctor, Nurse Practitioner or Physician Extender.
In-Home Care
We coordinate with your health care provider for services such as:
- Medication management and administration
- Lab draws
- Vital signs monitoring
- Wound care
- Central line dressing changes
- Assisting with assessment for in-home care givers
- Health service referrals
- Transportation to medical appointments
- Providing a second pair of eyes and ears during visit
Hospice and Palliative Care
Hospice care services include:
- Coordinate with your hospice care company
- Assist with referrals to hospice companies
- Provide extra care needs not provided by hospice company
- Educate family and care giver on hospice
- Assist with administering medications as-needed
- Provide your loved one with comfort and ease
Associated Care Coordinators is a fee for service company, not a third-party biller.
